About the opportunity
As a Care Coordinator within the Out of Home Care (OOHC) program, you will play a key role in facilitating and coordinating care and support for children and young people. You’ll work collaboratively with carers and families, a team of Therapeutic Specialists, external agencies and service providers, and the Department of Communities to deliver holistic, person-centred support.
In this role, you will implement individual support plans and coordinate external referrals where appropriate, ensuring each young person receives tailored and responsive care. You will take an integrated case management approach, grounded in a shared understanding of the risks and support needs of children and young people in out of home care.

About you - Key Selection Criteria:

  • An appropriate tertiary qualification in Psychology, Social Work, or a related discipline is preferred.
  • Demonstrated experience in a similar role in the out of home care sector in Western Australia.
  • Previous experience in a case management role.
  • Strong understanding of the policies governing the out of home care sector.
  • Ability to navigate complex situations and make informed decisions.
  • A knowledge of trauma, attachment and neurobiology frameworks and theories.
  • Substantial experience in the area of child abuse and/or in working with traumatised children.
  • Demonstrated commitment to working collaboratively and the capacity to negotiate and liaise with volunteers, including carers, Department of Communities, other agencies, and the community.
  • A sophisticated understanding of the complexity of the service system.
  • Personal values consistent with the values of the organisation.
  • A current driver’s licence and Working with Children check or equivalent.
